Case Description
Complaint: Requests for something fixed to replace his newly made dentures in other clinic. Can’t bite and eat well with the dentures. Most of the time not wearing the dentures due to discomfort, pain and af fected speech.
History of complaint: New maxillary full denture and mandibular partial denture were completed on 20 June 2022 in another clinic. Since then not wearing the dentures most of the time due to discomfort and pain. Affecting the life style significantly.
Extraoral examination:
Sunken peri oral facial region with loss of upper lip support.
Intraoral examination:
Maxillary arch:
– Fully edentulous.
– Healthy gum and palate.
Mandibular arch:
– Remaining 5 teeth from #42-#33, all are firm and healthy.
– Missing teeth #34-#37 and #43-#47.
Radiographic findings:
– Remaining five teeth with good prognosis with sufficient bone around these teeth.
– Very thin sinus floor on the left and right upper quadrants, with available bone heights less than 1.0 mm.
Treatment plannings:
1. Upper full-mouth dental implants rehabilitation
2. Bilateral sinus bone augmentation
3. Restoration of lower bilateral free end saddles region with multiple dental implants.
Management and definitive treatment:
19 July 2022:
– Eight dental implants were placed at maxillary arch with bilateral sinus bone augmentation.
– A total of 1.5 cc of allograft and OSTEON II sinus were used for each quadrant of the lateral sinus lift.
– Dentium DASK kit was used to performed the lateral window sinus bone augmentation.
– Six dental implants were placed at the mandibular arch, three on each quadrant.
13 February 2023:
– All dental implants were uncovered and placement of healing abutments.
– Post-treatment panaromic radiograph was taken.
4 April 2023:
– Close tray technique impression taking was done with stock trays and impregum.
2 May 2023:
– Lower left and right screw-retained crowns were fitted onto the dental implants
– Lower right three dental implants supporting four units screw-retained crowns.
– Lower left three units dental implants supporting three units crowns.
– Upper full-arch metal framework try-in. Upper framework was separated in three pieces: #16-#15, #14-#24, #25-#26- Another new MMR was taken with the framework in place.
23 May 2023:
– Completion of the case with all three screw-retained crowns/bridges were fitted onto eight dental implants on the maxillary arch.
– Post-treatment panaromic radiograpgh was taken.
